PATH-s是可自由访问的, evidence-based tool designed to assess caregiver’s preparedness to transition stroke patients home
的 Preparedness Assessment for the Transition Home after Stroke (PATH-s) instrument was developed by Michelle Camicia and Barbara 鲁茨 in response to the 皇冠6686会员登录 White Paper, “的 Essential Role of the Rehabilitation Nurse in Facilitating Care Transitions” (Camicia等., 2014). PATH-s是从理论框架发展而来的, “改善中风护理人员准备模式”(鲁茨等., 2017). 的 PATH-s instrument is a 25-item self-administered clinical assessment of caregivers developed to assess caregivers’ commitment and capacity prior to IRF discharge, before they have assumed the caregiving role (Camicia等., 2020). Camicia, 鲁茨, Harvath and Joseph (2021) identified that completion of an instrument such as 的路径-s may aid caregivers in recognizing potential issues and concerns about the caregiving role and provoke 期待 以及行动的线索 to begin addressing their concerns prior to IRF discharge. 的 PATH-s demonstrates good reliability and validity (Camicia, 鲁茨, Joseph, et al, 2021).
的 PATH-s instrument has been implemented with a corresponding catalogue of interventions (的 PATH2Caregiving Program). Several manuscripts are in development to illustrate the clinical benefits of this program. 进一步, a version of the instrument for use in the general inpatient rehabilitation population, 的路径, 的可靠性(Cronbach 's α=.90) and is available from the authors upon request (contact information provided below).
的 PATH-s has been translated into Chinese, Indonesian, Italian and Vietnamese languages. Validation of the instrument in these languages is underway.

2021年，path的作者Dr. 米歇尔·卡米西亚和医生. Barbara 鲁茨 presented a livestream webinar titled "Assessing Family Caregivers using 的路径-s仪器.在网络研讨会期间. Camicia博士. 鲁茨 describe the background, theoretical basis, and development of 的路径-s tool. Strategies for implementation in inpatient rehabilitation facilities are discussed and examples of successful integration of 的路径-s tool in the clinical setting are explained. Upon competing the activity, which offers the opportunity to earn 1.25 CNE学分，学员将能够:
- Recognize the importance of assessing caregiver preparedness
- Understand the theoretical foundation for 的路径-s instrument
- Identify strategies for administration of 的路径-s instrument
